Instead, your family can concentrate on supporting one another and celebrating your life, rather than be burdened with challenging decisions. Furthermore, recording your funeral wishes serves as a valuable tool to prevent family disputes from arising.

About the form

Whether you choose to use the downloadable form or the online form to record your funeral wishes, the questions are the same:

  • The first part of the form, My Personal Details, contains the information required by the Registrar-General of Births, Deaths, Deaths and Marriages (BDM). Please enter your details, as it will be helpful to those arranging the funeral when the time comes.
  • The second part, My Funeral Details, helps you record your preferences for the funeral arrangements.

You and your family are under no obligation to use Resthaven Funerals by recording your wishes with us: this is a free service.

Downloadable form

Once you have completed the form, it’s important that your family and close friends know about it so they are aware of your funeral wishes and instructions. You can also email a copy to us to keep on file: fd@resthavenfunerals.co.nz

We recommend that you keep a copy of your completed form with your important documents and paperwork, so it can be easily found in the future.
